Summary:
This paper is the culmination of dedicated efforts by the Federación Colombiana de Obstetricia y Ginecología (FECOLSOG), the Colombian Consensus for the Eradication of Rh Disease, a group of Colombian experts in obstetrics, gynecology, and maternal-fetal medicine.
Their goal was to establish comprehensive national guidelines and strategies, in Spanish, aimed at eliminating Rh disease in Colombia. To this end, their goal is to train healthcare professionals involved in the care of RhD-negative pregnant women, with the aim of standardizing the approaches to prevent maternal Rh sensitization and, if necessary, to diagnose, assess, and treat Rh disease, if it occurs.
Twenty-three expert panelists participated in developing the consensus, using three rounds of answering a questionnaire containing 8 domains and 22 questions, until a consensus threshold was reached. The 8 domains were: 1) RhD typing, 2) initial prenatal care for RhD-negative patients, 3) timing of indirect Coombs testing and titer determination, 4) sensitizing events, 5) administering Rh immune globulin, 6) middle cerebral artery Doppler ultrasound testing, 7) antenatal management of immunized mothers and anemic fetuses, and 8) timing for pregnancy termination in different settings. Based on answers to these questions and a review of international clinical practice guidelines, consensus statements were developed, including recommendations and their justification, which were also adapted for various local contexts.
